Here is something you may want to try. Just last week I had this issue, and 3 months ago I encountered it for the first time.
After lots of troubleshooting I found that the solution was to change the 3PDT wiring board. In both cases I had over soldered the foot switch to the board. I changed the board and it worked. I would rule out everything else and then try that.

    Hope it works.

    Solved.

Hi Billy, I appreciate your help. I found one of the Q5 socket pins was not soldered at all. This fixed the issue.

    Thanks so much. Sounds great.

Hi there, sorry for the late response, I put a 20K trimmer on the R5 position as suggested by Cybercow, that was genius. Here is an image I got

    got from        https://wiraelectrical.com/potentiometer-vs-rheostat/.

You can use a trim-pot by just using the A and B connections to dial in the desired ohms.

Thanks so Much. You are a genius. Just did what you suggested. Amazing, what a difference. i was able to adjust it perfectly. Wow. Thank you. My favorite build so far. AND I learned what “rheostat” is. Trully appreciate it.

It is on off on. It would be just the Op-amp clipping. You are 100% correct.
